This one is pretty good too:

http://www.telcodata.us/telcodata/telco

It seems 857-654 is used by many carriers so the next digit is necessary
to tell the carrier.

http://www.telcodata.us/telcodata/telco?npa=857&exchange=654

However it's all based out of Boston, MA

It looks like its most likely a T-Mobile mobile number, unless the
next digit is a 2 or 3, in which case its probably a Sprint mobile
number. The switch for this number is in the Norton Center in Boston,
MA.

I use http://www.areadecoder.com/ for quick lookups, but the best is
http://telcodata.us. It will give you a breakdown of each prefix, who
owns it, and in most cases the address (and a map) of the switch
location. Just go to http://telcodata.us and choose 'Area / Exchange'
under the 'Common Lookups' heading.

Thanks for the reference. Useful and interesting site.

Embedded in the internal exchange code for my city was two letters
that referred to the old original exchange name and rate zone for the
building which went back to manual or early dial days. Kind of neat
to see a legacy piece of technology continuing to this day. I wonder
if that applies to other big cities.

Also, I liked the cross-reference which showed other exchanges
operating out of the same building. Useful reference.
