Are you saying that your DHCP lease is 30 days but at some point
before that period you lose connectivity and can't reacquire an IP
address?

It might be time to start at the beginning. What are the lights on the
cable modem doing when you lose connectivity? When you can access it,
what do the modem's signal levels look like? Do they fluctuate over
time, such as time of day or from one day to the next?

The lights on the cable modem are as follows:
US: lit
DS: lit.
ethernet: blinking

I'll get the signal levels when I get back home.

Only 3 lights on the modem? What make/model is it? Does it even have a
web server available?

I came home to see none of the lights were on, except power.
I reset it and now I have everything.

Downstream power and SNR look acceptable to me. Upstream power looks
good, too.

The log entries, as bad as they look, seem to be pretty common, at
least in the modems that I've come across.

I'm not seeing anything obviously wrong so far, except that you said
that ALL of the lights were off when you got home, even the Ethernet
light. I could be wrong, but it seems like the Ethernet side of the
modem should stay lit as long as something is connected to it.

It might be tempting to go down and pick up a (rental) cable modem
from your local TWC office to use as a troubleshooting aid. If it
doesn't help, just return it.
