For OSX XMeeting is H.323 and Sip compliant. works pretty good for
free...Don't know any decent XP shareware stuff.

Some people like AccessGrid
AccessGrid requires older audiocards, no new drivers have been made.
Most Webcams seem to work for the video.
Accessgrid creates a video wall of talking heads.

Try Unreal Media Server http://umediaserver.net/. Free with up to fifteen
concurrent users.
