Does anyone know if this will work with the 4025 set (dni) or is it just for
the IP phones?

The 50000110 PKM 12 is for use with the 4000-series TDM sets, so yes it
will work with the 4025. However, you will also need a "9132-800-210-NA"
power adapter.
