So far there's appears to be an ECG Enhanced Communications Group
but they have a 59cents monthly fee,

You could perhaps select NO CARRIER and then either dial an access
code or use a prepaid calling card. SAMS sells prepaid AT&T cards
that do NOT expire or have monthly or higher first-minute costs for
3.4-cents per minute, and I'm sure you can find others in the same
price range.
